The deal, which has yet to be announced by the teams, is for three minor-leaguers: outfielder Aidan Smith, infielder Emilien Pitre and right-hander Gary Gill Hill.
Peralta, 30, is 5-9 with a 4.99 ERA in 22 starts for a bad Mets team. On a playoff-bound 2025 Brewers team, he was 17-6 with a 2.70 ERA and finished fifth in the NL Cy Young Award voting.
Over nine major-league seasons, Peralta is 75-51 with a 3.74 ERA.
The Rays have wanted to add a starter to provide depth to their rotation and have an opening as Shane McClanahan on Friday was placed on the injured list due to a back issue and will miss at least two starts.
